The Blue Cross Blue Shield Association published an analysis on 24 September 2026 estimating that hospitals billing inpatient stays as more medically complex added about $942 million to what its insurers paid over two years, measured against 2023. Billing for secondary conditions, the extra diagnoses listed beside the main reason for a stay, accounted for $653 million of that.
The association links the rise to AI that scans records and lab results for extra billable diagnoses. It also points to software that listens to the doctor's conversation with a patient and drafts the notes.
The share of members' inpatient claims billed as complex rose from about 37% in early 2023 to about 40% in late 2025. The treatment patients received did not change to match, the association said.
"If patients are truly sicker, we'd expect to see more treatment," said Luke Chalker, the association's senior vice president of product and data science.
The American Hospital Association calls such claims unsubstantiated. It said in July that patients are older and sicker and coding rules changed. It also said AI tools improve accuracy when a person checks the result. It counted a rise of about 5% in how sick its patients are since 2019.
