An NHS trust cannot say who opened 11 years of maternity records.

Nottingham University Hospitals NHS Trust overwrote its old maternity records database on 18 August 2026, The Register reported.

Staff were copying a radiotherapy database for reporting using pre-written instructions from another hospital system. According to the report, one setting was left unchanged, so the instructions pointed at the wrong database.

The database held information on women and babies treated between September 2011 and November 2022.

The trust recovered the clinical information needed for patient care, including test results and notes. Observations were also recovered. It said current maternity patients and services are unaffected.

What it could not recover was the full audit history showing which staff opened those maternity files across the 11 years. The trust described the incident as human error and said it was escalated within minutes of discovery.

The trust reported the matter to the Information Commissioner's Office and Nottinghamshire Police. The Register said police are assessing whether the loss affects Operation Perth, their investigation into maternity cases at the trust.
